Membership Form

Membership fee is $100 for Single and $150 for Family. Family membership is limited to Applicant and Spouse only. If you are mailing a check, make it payable to "Durga Temple". Please write “Membership Fee” on the memo line of your check. Except for new membership, yearly membership dues should be paid by March 31st of the current year.

Please note: A neat and thorough application ensures that our database is accurate. Please contact the temple office to report any changes to your membership data.
